The MHSAA has a NEW PHYSICAL FORM!

Please print this form and bring with you to the doctor.
  • For the 2011-12 School Year, physicals must be performed on or after April 15, 2011.
  • Physical forms must be COMPLETE.
  • Signed and dated by Doctor.
