Student Profile – form for seniors to provide information about themselves to be used by counselors and/or teachers to write letters of recommendation. 

Teacher Recommendation Request Form - Complete top two sections and give to core academic teacher at least 3 weeks in advance.

Transcript Release Form – for current seniors; gives Mercy High School permission to release transcripts to college and/or scholarship agencies. A form is REQUIRED for EACH college application, along with a $4.00 fee per request. 

College Interest Questionnaire – form for juniors to complete (with a parent) prior to making their Junior College Interview appointment with their counselor. CLICK HERE to read the letter mailed home explaining the purpose of the college planning meeting.
